What We’re Looking For

Trillium Health Resources has a career opening for a Care Manager-RB-BHT to join our team! The Care Manager-RB-BHT is responsible for providing complex care coordination and monitoring to members having a primary Intellectual or Developmental Disability, which may include a secondary mental health or substance abuse, and/or complex medical/physical health need. The Care Manager-RB-BHT assists with integrated care management through multi-disciplinary collaborations and referrals, including but not limited to: assistance with the TCM opt in/opt out process, completion of care needs screenings, 90-day transition plans, completion/updates of CMCAs and care plan development, and service coordination as needed/deemed appropriate.

This position has a primary focus on members that have been diagnosed with Autism Spectrum Disorder, particularly children (18mos – 15yo). The Care Manager-RB-BHT would be expected to work and be available at their remote location between the hours of 8:30 am – 5:00pm and be flexible to travel within the community to meet with members as well as attend other meetings as needed.

On a typical day, you might:

• Serve as the primary point of contact for assigned members, providing ongoing care management and advocacy.
• Conduct comprehensive assessments of member needs using person-centered planning methods and organizational tools.
• Develop, implement, and monitor individualized care plans in collaboration with members, families, and multidisciplinary support teams.
• Coordinate services across providers, agencies, and community resources to address member physical, behavioral, and social health needs.
• Ensure assessments and care plans are updated to reflect changes in member needs and circumstances.
