Role Description The Care Advocate is a full-time, hybrid role based in Mobile, AL, with some work-from-home flexibility. In this position, the Care Advocate supports members throughout their preventive health journey by coordinating appointments, explaining program services, and ensuring timely follow-up on diagnostics and care plans. The role involves regular communication with members, families, and clinicians to clarify recommendations, address questions, and encourage adherence to personalized health plans.
The Care Advocate maintains accurate documentation, updates member records, and collaborates with the clinical team to identify barriers to care and propose practical solutions. This role also includes educating members on available resources, monitoring engagement, and helping employers understand the program’s impact on employee well-being.
- Strong member-facing skills, including empathetic communication, active listening, and the ability to explain medical information in clear, accessible language.
- Experience in care coordination or patient advocacy, including scheduling, follow-up management, and collaboration with physicians and clinical staff.
- Proficiency with digital tools and electronic health or member record systems, along with basic data entry and organizational skills.
- Knowledge of preventive health, primary care, or chronic disease management, or willingness to learn and apply current medical science in a structured program.
- Ability to work effectively in a hybrid environment, manage time independently, and maintain confidentiality and compliance with healthcare privacy regulations.
- Prior experience in healthcare, social work, case management, or a related field is preferred.
- Associate or bachelor’s degree in a health-related, social services, or human services discipline is beneficial but not required with relevant experience.
