Position Overview

Assist adults in home-based community settings by providing one-on-one support to promote independence and community participation.

• Deliver personal care, socialization, health maintenance, medication support, and skill development as per individual plans.
• Assist with daily activities such as housekeeping, transportation, recreation, shopping, and community integration.
• Accurately document progress, maintain confidentiality, and support individuals' rights.
• Monitor health, support medical appointments, and assist with medication administration.
• Ensure safety, cleanliness, and proper operation of medical equipment and vehicles.
• Maintain professional relationships with individuals, families, and team members.
• Participate in ongoing training, adhere to safety policies, and work collaboratively with colleagues.

• High school diploma or equivalent preferred; six months human services experience desirable.
• Valid driver's license, clean driving record, and ability to transport individuals safely.
• Basic computer skills.
• Ability to perform physical tasks, including lifting up to 100 pounds occasionally.
• Commitment to safety, confidentiality, and respectful communication.

• Completion of mandated training and background checks.
• Must be 18 years or older.
• Willingness to work in a team environment and adapt to various community and home settings.
