The Care Manager is a part-time, hybrid role based in the Greater Seattle Area, with some work-from-home flexibility. This role coordinates and oversees care plans for clients, ensuring services align with individual needs, preferences, and safety requirements. Day-to-day responsibilities include conducting assessments, monitoring client progress, and collaborating with healthcare providers, family members, and community resources. The Care Manager provides guidance on available services, supports transitions of care, and advocates for clients’ well-being and rights.

Additional tasks may include maintaining accurate documentation, participating in training and quality improvement activities, and supporting Softera’s homecare and emerging home health initiatives.

• Strong case management skills, including assessment, care planning, coordination, and follow-up.
• Experience in elder care and familiarity with age-related needs, supports, and resources.
• Background in social services, with knowledge of community programs and support networks.
• Foundational understanding of medicine or clinical concepts relevant to homecare and home health.
• Ability to provide training and guidance to caregivers, clients, and families on care practices and resources.
• Excellent communication, active listening, and interpersonal skills with diverse populations.
• Strong organizational skills, attention to detail, and comfort with electronic records and basic technology.
• Experience in homecare, care coordination, or related healthcare roles; relevant certification or licensure is a plus.
• Ability to work both independently and collaboratively in a hybrid setting within the Greater Seattle Area.
