Reports to the Home Health & Hospice Director

The Clinical Community Health Coordinator serves as the vital link between clinical care, community engagement, and operational growth. This role is responsible for ensuring patient safety and continuity of care through five key areas of focus:

• Clinical Continuity: Acting as the primary liaison within area facilities and with referral sources to provide expert consultation on Home Health and Hospice services, ensuring seamless care coordination and "warm handoffs" for transitioning patients.
• Strategic Business Development: Driving agency growth by building high-trust relationships with physicians and referral sources through targeted marketing, educational outreach, and proactive account management.
• Community Leadership: Serving as the agency’s lead ambassador in Carteret County by coordinating major fundraising events and providing public education to demystify home-based clinical services.
• Regulatory Program Management: Overseeing all aspects of the Hospice Volunteer Program, ensuring full compliance with Medicare’s 5% cost-savings mandate through effective recruitment, training, and database management.
• Organizational Integration: Facilitating high-level communication across the interdisciplinary team and Carteret Health Care to maintain professional standards and fulfill the agency’s mission.

Professional

Maintain a current license to practice as a Registered Nurse in North Carolina.

Possesses a valid NC Driver’s license and reliable independent transportation.

Home Health & Hospice and/or healthcare sales/marketing experience preferred.

Possess AHA BLS Healthcare Provider Certification.

Proven experience in developing and delivering patient-centered health education in an accessible and understandable manner.

Strong cultural competence with the ability to adapt to communication styles to meet diverse cultural, linguistic and educational backgrounds of community members.

Sensitivity to cultural nuances and an understanding of how cultural factors impact health beliefs and behaviors.

Proven experience in collecting, organizing, and managing diverse demographic and health-related data sets.

Strong analytical proficiency skills to extract meaningful insights from data sets identifying trends, patterns and areas of improvement to draw evidence-based conclusions.

Demonstrate knowledge of medical procedures, safety protocols, best practices, and regulatory compliance standards.

Exhibits exceptional organizational and verbal/written communication skills, with the ability to multitask and manage competing priorities.

Self-driven and ability to work independently, as well as collaboratively as part of a team.

Possess proficiency in Google Suite & Meditech & Matrix Care software.
