The Health Lifestyle Coach provides nutrition education and lifestyle coaching to patients, working closely with the multidisciplinary healthcare team to help improve health outcomes for patients diagnosed with lifestyle-related chronic diseases related to diet and physical activity. Primary responsibilities include providing classes and individual coaching sessions to patients in-person or through telehealth that are evidence-based and patient-centered, managing the daily schedule to maximize patient care.

Additional responsibilities may include assisting with projects, presenting on services to the healthcare team, and training new staff.

• Degrees:
  Bachelor's Degree in a health related field required (Degree in nutrition or dietetics preferred)

  Certifications:
  
  Certified as a health or wellness coach through a National Board of Health and Wellness Coaches (NBHWC) approved program; required within six (6) months if held at time of appointment.

• Work Experience:
  
  One (1) year work experience providing nutrition education and/or lifestyle coaching to patients or in the community preferred
