About the Role:
Local Hospice Company is looking for a compassionate and dedicated Hospice Medical Social Worker (MSW/LSW) to join our caring team. In this meaningful role, you'll provide vital emotional, social, and resource support to patients and families navigating end-of-life care. Responsibilities:- Conduct psychosocial assessments to evaluate patient and family needs, strengths, and coping abilities
- Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team
- Provide counseling and emotional support to patients and families facing terminal illness and grief
- Connect patients and families with community resources, financial assistance programs, and support services
- Assist with advance care planning, including advance directives and end-of-life decision-making
- Facilitate family meetings and coordinate communication between patients, families, and the care team
- Provide bereavement support and follow-up services to families after the loss of a loved one
- Maintain accurate and timely clinical documentation in compliance with hospice regulations and standards
- Participate in total plan of care development, interdisciplinary team (IDG) case conferences, and comprehensive discharge planning as needed.
- Master's degree in Social Work (MSW) from an accredited program required
- Current LSW or LCSW licensure in the state of Illinois
- Prior experience in hospice, palliative care, home health, or a related healthcare setting preferred
- Strong knowledge of end-of-life care, grief counseling, and community resource coordination
- Excellent interpersonal and communication skills with a compassionate, patient-centered approach
- Ability to work independently and collaboratively within an interdisciplinary care team
- Valid driver's license and reliable transportation for patient home visits in the Saint Charles, IL area
