Remedy Home Health Care is actively seeking a licensed Physical Therapist (PT) to join our growing Home Health team in Los Angeles County
. This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes.

As a Home Health Physical Therapist
, you will work with senior care populations
, including geriatric patients recovering from illness, injury, or surgery. You’ll deliver skilled therapy services in home-based, assisted living, and community care settings
, helping patients regain independence and improve their quality of life.

As a Physical Therapist in Home Health
, you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients
.

• Conduct in-home physical therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
• Improve strength, balance, mobility, and functional outcomes
• Provide therapy in home health, assisted living, and community care settings
• Educate patients and families on fall prevention and safe mobility techniques
• Document visits using Well Sky (Kinnser) EMR
• Collaborate with interdisciplinary home health teams

• Licensed Physical Therapist (PT) – California (Required)
• CPR Certification (AHA approved)
• Minimum 1 year of clinical experience (Home Health preferred)
• Experience in senior care, geriatrics, rehab, or community-based care preferred
• Strong clinical, communication, and patient education skills
• Valid driver’s license, reliable transportation, and auto insurance

• Flexible Per Diem / PRN, Part-Time, or Full-Time opportunities
• Hands-on experience in home health and rehabilitation care
• Work with geriatric and senior care populations
• Independence in a mobile, community-focused role

Pay-Per-Visit

$120 - $150 USD
