RN Care Manager Supervisor

The primary function of the position is support the operations of the Care Management and Service Coordination program (Care Management); complete required assessments using applicable forms; providing pre-enrollment education on behalf of third party insurance programs; and assuming care management responsibilities as assigned. This position requires field work as well as in office work.

Requirements?

NYS RN licensure is required, and experience in community health roles certainly desired (but not required). Supervision experience is preferred.

The position requires travel to client homes (local); along with work from home.
