• Serve as the primary point of contact for families participating in the Arizona Early Intervention Program (AzEIP), guiding them through the process of accessing services and resources.
• Coordinate evaluations, assessments, and service delivery within the Team-Based Early Intervention Model to ensure each child receives appropriate care.
• Assist families in accessing early intervention services, funding sources, and community support.
• Facilitate the development and ongoing review of the Individualized Family Service Plan (IFSP) to ensure it reflects each child's unique needs.
• Support families in understanding their rights, procedural safeguards, and the resources available to them.
• Collaborate with other professionals, including OT, PT, SLP, DSI, and other team members, to deliver coordinated care.
• Maintain accurate documentation and communication with all involved professionals to ensure seamless coordination and support for families.

• Bachelor's degree in early childhood, early childhood special education, or family studies (master's degree preferred)
• Experience or strong interest in early intervention or family-centered services
• Bilingual in English and Spanish preferred (not required)
• Valid CPR certification and Fingerprint Clearance Card (or willingness to obtain)
• Strong communication, organization, and time management skills
• Empathetic, flexible, and dedicated to helping families access the resources they need
