This is a full-time hybrid Consultant Pharmacist role based in Salt Lake City, UT, with flexibility for some work from home. The Consultant Pharmacist will perform comprehensive medication regimen reviews, evaluate drug therapy for efficacy and safety, and identify opportunities to improve pharmacotherapy outcomes. Daily responsibilities include reviewing patient charts, documenting clinical recommendations, collaborating with prescribers and nursing staff, and ensuring compliance with state and federal pharmacy regulations.

The role also involves preparing consultative reports, participating in quality assurance activities, supporting survey readiness, and educating facility staff on best practices in medication management. The Consultant Pharmacist will use electronic health records and other clinical systems to track interventions and communicate findings, while maintaining accurate and timely documentation.

• Active and unrestricted Pharmacist license in Utah (or eligibility for licensure), with strong knowledge of clinical pharmacotherapy and medication management.
• Experience in long-term care, ambulatory care, hospital, or consulting pharmacy settings, with familiarity in regulatory and compliance requirements (e.g., CMS, state boards of pharmacy).
• Proficiency with electronic health records, clinical documentation systems, and basic office software; comfort working in both on-site facility and remote environments.
• Strong analytical, critical thinking, and problem-solving abilities to review complex medication regimens and develop clear, evidence-based recommendations.
• Excellent written and verbal communication skills, including the ability to present findings, educate interdisciplinary teams, and build collaborative relationships with providers and staff.
• High level of professionalism, integrity, and attention to detail, with effective time management and organizational skills to handle multiple facilities or caseloads.
• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution; relevant certifications (e.g., BCPS, BCGP) are a plus.
• Commitment to continuous learning, staying current with clinical guidelines, therapeutic advances, and changes in regulatory standards.
