Physical Therapy Assistant About the Role
Physicians Choice is seeking a compassionate, dependable, and motivated Physical Therapy Assistant (PTA) to join our home health team supporting patients in the Portland, Oregon area. In this full-time, home-based role, you will deliver skilled physical therapy services in patients’ residences under the direction and supervision of a licensed Physical Therapist. Success in this position means helping patients improve strength, mobility, independence, and safety—while providing patient-centered care, communicating clearly with the interdisciplinary team, and maintaining accurate, timely documentation to support continuity of care.
Duties & Responsibilities- Provide skilled physical therapy interventions according to the established Plan of Care and under the direction of the supervising Physical Therapist.
- Implement therapeutic exercises, gait training, transfer training, balance activities, strengthening, mobility training, and other interventions ordered by the Physical Therapist.
- Support patients in improving functional mobility, strength, endurance, balance, and independence with activities of daily living.
- Educate patients, family members, and caregivers on therapeutic exercises, mobility techniques, fall prevention, assistive device use, and home safety.
- Monitor patient response to treatment and promptly communicate significant changes in condition, functional status, or progress to the supervising Physical Therapist and care team.
- Reinforce home exercise programs and provide ongoing patient and caregiver education between visits as appropriate.
- Identify and report potential safety concerns, changes in the home environment, and barriers that may affect progress toward therapy goals.
- Coordinate care and communicate effectively with the interdisciplinary team, including Physical Therapists, nurses, occupational therapists, speech-language pathologists, and home health aides.
- Complete accurate, thorough, and timely clinical documentation in accordance with agency policies, payer requirements, and applicable state and federal regulations.
- Maintain appropriate visit frequency, follow physician/provider orders, and communicate scheduling or missed-visit concerns to the appropriate team members.
- Assist with discharge planning by communicating patient progress toward established therapy goals.
- Follow HIPAA, infection-control, patient-safety, and emergency-preparedness policies and procedures.
- Graduate of an accredited Physical Therapist Assistant program.
- Current and unrestricted Oregon Physical Therapist Assistant license or eligibility for Oregon licensure.
- Current CPR/BLS certification.
- Valid driver’s license, reliable transportation, and automobile insurance as required for travel to patient homes.
- Ability to safely perform the physical requirements associated with providing therapy in a home health environment.
- Strong communication, organizational, and time-management skills.
- Ability to work independently while remaining an effective member of an interdisciplinary care team.
- Comfortable traveling throughout the Portland metropolitan area and surrounding service territory.
- Knowledge of fall prevention, home safety, gait training, and functional mobility (preferred, but demonstrated competence is required).
