Blue Kite Wellness provides accessible, compassionate, individualized, and holistic mental health services to children, teens, adults, and families. We are seeking an Intensive Home-Based Clinician (IHBC) to support children, youth, and families participating in the Illinois Pathways to Success program.
The IHBC provides individualized, short-term behavioral health services in the settings where children and families live and grow. This role develops assessment-informed intervention plans, provides therapeutic services focused on symptom reduction and improved family functioning, and offers clinical direction to Intensive Home-Based Support (IHBS) staff.
The clinician works collaboratively with youth, caregivers, Care Coordinators, Child and Family Team members, and other authorized providers. Services are strengths-based, family-centered, solution-focused, and informed by Managing and Adapting Practice (MAP) and other HFS-approved approaches.
What You’ll DoProvide Intensive Home-Based Clinical services to eligible children, youth, and families in accordance with authorized service plans and Illinois Pathways requirements.
Use the youth’s IM+CANS, Functional Behavioral Assessment, clinical observations, family input, and Child and Family Team recommendations to develop and adjust individualized clinical interventions.
Deliver individual, family, caregiver, and cross-system therapeutic interventions that support symptom reduction, emotional and behavioral functioning, family stability, and the youth’s ability to remain safely at home and in the community.
Strengthen caregiver capacity, family communication, problem-solving, coping, emotional regulation, and behavioral-management skills.
Provide clinical direction and consultation to assigned IHBS staff, including communicating intervention goals, reviewing progress, and modifying strategies when clinically appropriate.
Participate as an active member of the Child and Family Team and collaborate with caregivers, Care Coordinators, schools, crisis-response providers, and other authorized supports.
Support crisis prevention, safety planning, de-escalation, and post-crisis follow-up within the clinician’s scope of practice.
Monitor client progress, identify emerging needs or safety concerns, and recommend appropriate changes to services and clinical plans.
Complete timely, accurate, objective, and clinically appropriate documentation in accordance with Blue Kite Wellness, payer, and Illinois HFS requirements.
Participate in supervision, case consultation, team meetings, required training, and continuing professional development.
Required Qualifications
Meets current Illinois requirements for a Qualified Mental Health Professional (QMHP) or Licensed Practitioner of the Healing Arts (LPHA), including all applicable education, licensure, supervision, credentialing, and scope-of-practice requirements.
Master’s degree in social work, counseling, psychology, or a related behavioral health field from an accredited institution.
At least two years of progressively responsible behavioral health experience, including substantial experience working with children, adolescents, and families.
Strong clinical assessment, engagement, treatment-planning, family-intervention, crisis-prevention, de-escalation, documentation, and care-coordination skills.
Ability to work independently in home and community settings while collaborating effectively with a multidisciplinary team.
