RN Care Manager; Mississippi

The RN Care Manager at Molina Healthcare provides support for care management and coordination activities, collaborating with a multidisciplinary team to ensure integrated delivery of member care. • Pay: $23 - $51/hour
• Location: Remote, US
• Schedule: Full Time

• Complete comprehensive assessments of members per regulated timelines.
• Develop and implement care coordination plans in collaboration with members and healthcare professionals.
• Conduct telephonic, face-to-face, or home visits as required.
• Perform ongoing monitoring of care plans to evaluate effectiveness and suggest changes.
• Maintain an ongoing member caseload for regular outreach and management.
• Promote integration of services for members, including behavioral health and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing to educate and support members.
• Assess for barriers to care and provide assistance to address concerns.
• Conduct medication reconciliation as needed.

• Education: Registered Nurse (RN) License must be active and unrestricted in state of practice.
• Experience: At least 2 years experience in healthcare, preferably in care management or a medical/behavioral health setting.
