Safety‑Sensitive Designation

Position is safety‑sensitive, requiring driving and sustained alertness. Must comply with drug/alcohol testing and fitness‑for‑duty evaluations as per policy.

Summary

Arkansas Provider Coalition (APC) provides Care Coordination for Summit Community Care, assisting individuals with developmental disabilities and/or behavioral health diagnoses to maintain health and independence. The role ensures patient‑centered care through service referrals, follow‑ups, and navigation.

Key Responsibilities
  • Develop, document, evaluate, monitor, and modify members’ annual Person‑Centered Service Plans (PCSP) on an ongoing basis.
  • Ensure members’ health, safety, and service needs are met by working directly with members, families/guardians, providers, and support circle.
  • Maintain monthly contact with members, families, providers, community resources, stakeholders, and other professionals.
  • Conduct in‑person visits at least once every quarter; more frequent visits for complex cases.
  • Use telephone at least once a month and virtual meetings via company‑provided laptop.
  • Document activities and member information in the electronic medical records system (EMR) within 24 hours.
  • Comply with core metrics and regulatory standards for assigned members.
  • Answer questions and provide information over phone, email, and in person.
  • Model continuous learning and performance improvement; incorporate new learning into practice.
  • Maintain compliance with Conflict‑Free Case Management rules and all applicable Medicaid regulations.
  • Ensure confidentiality and integrity of APC; comply with HIPAA and APC policies.
  • Operate company‑issued laptop, smartphone, and other software as required.
  • Use personal automobile for travel to meetings, member homes, provider offices, conferences, and training.
  • Maintain a secure home office with reliable internet.
Knowledge, Experience, Skills, and Abilities
  • Minimum one year beginner‑level experience with Microsoft Excel, Outlook, & Word and a bachelor’s degree in social work, psychology, sociology, or related health field; OR two years college coursework in relevant field plus one year experience with developmental/behavioral health individuals.
  • Knowledge of computer and business software applications, databases, email, presentations, spreadsheets, word processing.
  • Knowledge of community and state resources to integrate services into member care plans.
  • Proficiency with electronic filing systems and record‑keeping.
  • Skills in utilizing business software to develop, document, and manage PCSP and EMR.
  • Ability to operate standard office equipment and an automobile for multitasking.
  • Ability to use iPhone and relevant applications for communication and record management.
  • Strong communication skills in English and/or Spanish; effective in diverse settings.
  • Uphold confidentiality and HIPAA regulations.
  • Comply with Conflict‑Free Case Management and Medicaid regulatory standards.
Mandatory Expectations
  • Hold valid Arkansas Class D (Non‑Commercial Vehicle) Driver’s License during employment.
  • Maintain reliable, insured, licensed vehicle for reimbursable business travel.
  • Provide proof of current, valid automobile insurance before employment and upon request.
  • Commute reliably to/from member homes, provider offices, and meeting spaces as required.
  • Pass required pre‑employment screenings: criminal background, drug/alcohol, motor vehicle report, child/maltreatment registries, sex offender check, and verification of credentials.
