The Collaborative Care Clinician II at Concert Health is an independently licensed behavioral health clinician who delivers evidence-based interventions within the Collaborative Care Model. You will work remotely with pediatric and/or adult populations, coordinating with primary care teams to promote symptom reduction and optimal outcomes.
Requirements include a Master’s in Social Work and California licensure (LCSW/LPCC), plus 1–5 years of licensed experience and reliable video-capable internet
