Remote APRN – Dementia Care Lead Equity

PocketRN is hiring a Remote Advanced Practice Provider to support the dementia care program (GUIDE). You will review clinical documentation, attestations, and support timely, high-quality care, coordinating with care navigators, clinicians, and patients.

This is a fully remote role with requirements for active APRN licensure and robust telehealth capabilities. Expect to work with CMS-aligned dementia care criteria, maintain high documentation standards, and ensure timely billing and compliance
