CA Licensed Clinician - ACSW, AMFT or APCC - Behavioral Health; Fully Remote

• Provide evidence-based interventions via the Collaborative Care Model to individuals with mild to moderate symptoms.
• Work with pediatric and/or adult populations to promote symptom reduction and optimal patient outcomes.
• Report to the Clinical Excellence Manager and collaborate with a care team.

• Screen patients for common behavioral health and substance use disorders using PHQ9, GAD7, and Columbia Suicide Risk Assessment.
• Monitor patients for changes in symptoms and treatment side effects, and support medication management prescribed by PCPs.
• Track patient outcomes using a registry and document progress in Salesforce and the electronic health record.
• Base salary ranges from $56,707.60 to $ depending on licensure and geographic location.
