Remote Aging Care Coordination Specialist

Sailor Health is hiring a full-time, US-based remote Care Coordination Case Manager. You will own continuity of care for patients needing higher levels of care beyond virtual outpatient therapy, including high acuity and complex medical or social needs.

Responsibilities include identifying appropriate patients, assessing needs, planning transitions of care, and coordinating referrals and appointments. A Bachelor’s in a related field is required;
Medicare experience is preferred.
