Care Navigator; Fully Remote

• This is a full-time, temporary remote position focused on high-volume outreach and end-to-end care coordination.
• You will help patients navigate complex systems, overcome barriers, and access care that improves their health and well-being.

• Conduct patient assessments to understand clinical, social, and logistical barriers to care.
• Educate patients on available healthcare services and community resources, and follow up to ensure completion of care.
• Maintain accurate documentation of all interactions in accordance with HIPAA and privacy standards.

• 4+ years of experience in care navigation, coordination, social work, call centers, or patient outreach.
• Strong critical thinking, emotional intelligence, and ability to work independently in a fast-paced environment.
• Flexibility, humility, and a sense of humor are key personality traits for thriving on this team.
