Speech Pathologist, Rehabilitation
This full-time Speech Language Pathologist will deliver home-based speech-language services to patients in Coeur d'Anle, Idaho. The role focuses on improving communication, cognition, swallowing, and overall quality of life through individualized therapy. The position supports a patient-centered, interdisciplinary care team.
Key Responsibilities:Provide speech-language pathology services to patients in their homes
Evaluate patients with speech, language, communication, cognitive, and swallowing disorders
Develop individualized treatment plans based on patient needs, diagnoses, and functional goals
Deliver therapeutic interventions to improve communication, cognition, and swallowing function
Assess and treat patients with neurological, medical, and age-related conditions
Educate patients, families, and caregivers about treatment plans, compensatory strategies, and home exercise programs
Recommend communication strategies and adaptive equipment when clinically indicated
Monitor patient progress and modify treatment plans as needed
Complete accurate and timely clinical documentation per home-health standards
Collaborate with physicians, nurses, occupational therapists, physical therapists, social workers, and other interdisciplinary team members
Participate in care coordination and communicate changes in patient condition as appropriate
Maintain a professional and compassionate approach while working independently in patients' homes
Master's degree in Speech-Language Pathology from an accredited program
Current Idaho Speech-Language Pathology license or eligibility for Idaho licensure
Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) (required according to licensing standards)
Valid driver's license and reliable transportation
Ability to travel to patient homes throughout the assigned service area
Strong clinical assessment, documentation, communication, and organizational skills
Skills:
Experience providing speech therapy in a home health setting
Experience treating adult and/or geriatric patients
Knowledge of dysphagia and swallowing disorders
Experience with cognitive-communication disorders
Experience with neurological conditions and rehabilitation
Strong patient and caregiver education skills
Ability to work independently while maintaining communication with the interdisciplinary care team
Excellent time management and scheduling skills
Commitment to patient-centered care and functional outcomes
Current ASHA certification (preferred)
Current CPR certification (preferred)
Previous home health speech therapy experience (preferred)
You hold an ASHA CCC and Idaho license. You can travel independently to patients' homes statewide. You excel at documenting, coordinating care, and patient education
