Community Health and Wellness of Greater Torrington, Inc. seeks a Community Health Worker (CHW) to assist patients and families in navigating community and social services, promoting healthy behaviors, and coordinating care with providers and the care management team.
The CHW will participate in outreach, home visits, health screenings, and referrals, while maintaining patient records and supporting the care team in a collaborative environment. English proficiency required;
Spanish preferred.
