Location:Sacramento, CA - Hybrid. This is a field-based home health role primarily serving patients in Davis and surrounding communities, with documentation completed from the field.
Note:Candidates must have an active California Physical Therapist (PT) license. Home health experience, 1+ year of Physical Therapist experience, a Master's or Doctorate degree in Physical Therapy, BLS/CPR certification, a valid driver's license and reliable transportation, HCHB/Homecare Homebase experience, and OASIS experience are preferred.

Job Description- Evaluate patients in their homes and assess mobility, strength, balance, coordination, pain, and functional limitations.
- Develop individualized physical therapy treatment plans based on each patient's goals and clinical needs.
- Provide therapeutic exercises and interventions designed to improve mobility, strength, function, and independence.
- Educate patients and caregivers on exercises, safety precautions, fall prevention, and self-care strategies.
- Monitor patient progress and adjust treatment plans as needed.
- Collaborate with nurses, physicians, therapists, caregivers, and other members of the interdisciplinary team.
- Complete accurate and timely clinical documentation.
- Travel throughout Davis and surrounding communities to provide high-quality in-home physical therapy care.
- Active California Physical Therapist (PT) license required.
- Home health Physical Therapist experience preferred.
- 1+ year of Physical Therapist experience preferred.
- Master's or Doctorate degree in Physical Therapy preferred.
- Current BLS or hands-on CPR certification preferred.
- Valid driver's license and reliable insured vehicle preferred.
- HCHB/Homecare Homebase experience preferred.
- OASIS experience preferred.
