Corewood Care is a home care agency serving older adults and their families in the Washington, DC metro area. Our care management team helps clients stay independent and safe at home, and helps families make decisions during complicated and often stressful times. We are hiring for the long term.
What We Offer- $65 per hour for care management work
- Paid documentation and correspondence. Your time on notes, reports, calls, and emails is paid, not just client visits.
- $50 per hour paid travel time between clients and appointments
- Time and a half for any work outside regular business hours (8:30 AM to 6:00 PM, Monday through Friday), including evenings and weekends

- Independence. Most of your work is self-directed, with a Director of Care Management to collaborate with.
You will act as advisor, advocate, and coordinator for older adults and their families as they navigate healthcare, community resources, and long-term care options. You will:
- Conduct in-home or virtual assessments for new clients, including a comprehensive biopsychosocial assessment
- Develop a Plan of Care with the Director of Care Management, the client, and the family
- Write a Recommendations Report based on the client's needs and wishes, and present it to the client, family, and/or other professionals
- Make follow-up visits, in person or virtual, to carry out and adjust the Plan of Care and to assess safety, health, and cognitive risks
- Serve as advocate and liaison to medical providers and other members of the health care team
- Escort and drive clients to physician and other appointments as needed
- Provide supportive counseling, family support, and mediation as needed
- Assist with or coordinate around guardianship/conservatorship and fiduciary needs as they arise
We value substantial hands-on experience over any single credential. Our team members come from many different backgrounds, and we've worked successfully with people from many fields. Backgrounds that often translate well include nursing, social work, counseling, psychology, gerontology, therapy (OT, PT, recreational), home health or home care management, senior living, healthcare administration, and nonprofit or community-based aging services. These are examples, not requirements.
If you have worked with older adults or other vulnerable populations and have the qualities below, we want to hear from you.
You likely have:
- 4 + years of experience working directly with older adults or other vulnerable populations
- Working knowledge of the senior care continuum: independent living, assisted living, home health, rehabilitation hospitals, skilled nursing, and aging in place
- Experience coordinating services and supporting families through complex situations
Preferred: home visit experience, in-home service experience, and care coordination or case management experience.
Strengths that matter most: active listening, emotional intelligence, sound judgment, clear communication, strong professional boundaries, and the ability to work independently across multiple clients.
Location and TravelMost of our clients are in Montgomery County, MD and Washington, DC, and candidates in those areas are preferred. We also welcome applicants from Frederick, Howard, and Prince George's Counties and Northern Virginia. You'll need reliable transportation and a willingness to travel to client homes and community locations. Much of the remaining work can be done from a home office.
