Homeside Rehab is seeking licensed Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide home-based therapy services to patients in Borough Park, Brooklyn
, specifically within the 11219 and 11204 ZIP codes.

This is an excellent opportunity for therapists looking to add flexible, local home care cases to their existing schedules while working with a supportive clinical and administrative team.

• Provide skilled physical therapy services in the patient's home

• Evaluate patient needs and develop appropriate treatment plans, as applicable to the PT role

• Provide treatment according to the established plan of care

• PTAs will provide treatment under the supervision of a licensed Physical Therapist

• Monitor patient progress and communicate changes to the clinical team

• Educate patients and caregivers regarding treatment plans and home exercise programs

• Complete accurate and timely clinical documentation

• Maintain professional communication with patients, caregivers, and the Homeside Rehab team

• Active New York State Physical Therapist (PT) or Physical Therapist Assistant (PTA) license

• Minimum 1 year of experience preferred

• Home care experience preferred, but not required

• Reliable transportation

• Strong communication and documentation skills

• Ability to work independently in a home care setting

• Dependable and able to maintain a consistent patient visit schedule

• Competitive pay: $45–$75 per hour
