Description:
Under the direction of a physician order, plans and implements specific physical therapy evaluation and treatment plan programs.
The treatment is based on the patient’s physical, developmental, and psychosocial needs.
Provides advisory service to other team members to ensure quality patient care.
Functions under the direct supervision of the Director and Assistant Director.
Performs related duties as assigned.
Responsibilities:
Performs Evaluation to include Orders, Diagnoses, and History.
Gathers Subjective Findings including Patient Report of Pain, Patient/Family Goals, Prior Level of Function, Mental Status, and Discharge Plan.
Assesses Motor and Sensory Function including but not limited to Range of Motion, Muscle Tone, Strength, Coordination, Balance, Fall Risk, and Sensation and Edema.
Evaluates Functional Mobility and Transfers including Home/Work/School environment.
Assesses Gait/Wheelchair Mobility including Distance, Device, Assistance, and Deviation.
Monitors Endurance including Duration of Activity and Vital Signs.
Develops Plan and Recommendations including Patient-centered, Function Focused, Measurable Goals, and Patient/Family Education.
Provides Treatment to include Interventions to meet Functional, Psychosocial, Cultural, Educational, and Spiritual needs.
Utilizes Therapeutic modalities including Ultrasound, Hot Packs, Electrical Stimulation, Whirlpool, and Transdermal Medication.
Conducts Therapeutic interventions including Therapeutic Procedures, Neuromuscular Reeducation, Gait Training, Manual Therapy, Therapeutic Activities, Wheelchair Management, Wound Care, Orthotic Management, and Prosthetic Management.
Demonstrates effective interdisciplinary collaboration and documentation of said collaboration.
Involves in Discharge Planning.
Educates Patient/Significant Other/Caregiver on Home Program and Equipment.
Recommends Follow-up Care including Home Program, Home Health Care, Skilled Nursing, or Outpatient Therapy.
Completes all documentation in a timely and thorough manner in accordance with regulatory guidelines.
Meets established departmental productivity standards, conserves resources, and recognizes the value of cost containment while maintaining quality standards.
Consistently utilizes service techniques in establishing and maintaining a positive relationship with all customers, including co-workers.
Demonstrates a commitment to excellence of care by advancing one’s knowledge base through departmental and non-departmental venues.
Assumes responsibility and accountability for their level of work and adherence to competency standards.
Pursues opportunities for educational advancement and meets departmental continuing education requirements.
Provides effective supervision for students during clinical rotations.
Engages in process improvement activities which enhance patient outcomes and compliance with regulatory bodies.
Performs additional duties as directed.

Must have general knowledge of evaluation and treatment principles for a diverse range of diagnoses.

Physical Requirements:
Patient care may include heavy lifting and extensive bending and standing.
Physical demands involve pushing and pulling weights, gripping, pinching, kneeling, crouching, and crawling.
Competency in equipment used for the delivery of modalities is required.
