You will conduct monthly outreach, provide health coaching, and collaborate with clinical teams to close gaps in care while maintaining HIPAA-compliant documentation.
The role requires a CMA/RMA/CCMA/NCMA/CMAC or CPhT certification, at least 1 year of clinical experience, and proficiency with EMR systems and multiple technology platforms.
