We don’t simply add behavioral health services alongside primary care. We help practices build a more integrated model in which behavioral health clinicians, primary care providers, and psychiatric consultants work together to identify needs earlier, track outcomes, and adjust care when patients are not improving.

The Role

As a Behavioral Care Manager
, you will provide direct individual behavioral health treatment to patients while working virtually alongside primary care teams within the Collaborative Care Model (CoCM).

This is a hands-on clinical role. You will meet one-on-one with patients, provide brief, evidence-based therapy and behavioral interventions, monitor symptoms and treatment progress, and coordinate closely with primary care providers and psychiatric consultants.

While care coordination is an important part of the role, this is not primarily an administrative care-management position
. You will maintain your own patient caseload and provide direct therapeutic support as part of an integrated care team.

This is individual therapy delivered differently. Rather than traditional long-term psychotherapy, BCMs provide focused, evidence-based treatment in an integrated primary care setting, emphasizing measurable outcomes, timely follow-up, and close collaboration with the patient’s broader healthcare team.

This role may be a great fit if you enjoy direct patient care and value collaboration, measurement-based treatment, and close work with medical providers.

What You’ll Do
  • Provide individual behavioral health therapy and brief evidence-based interventions to patients
  • Maintain and manage an assigned patient caseload
  • Conduct behavioral health screenings and assessments using tools such as PHQ-9, GAD-7, and C-SSRS
  • Develop and update individualized treatment plans based on patient needs and measurable outcomes
  • Use evidence-based approaches such as CBT, Behavioral Activation, Motivational Interviewing, and Problem‑Solving Treatment
  • Monitor patient progress, including progress toward symptom reduction goals
  • Partner with primary care providers to coordinate behavioral health treatment within the patient’s overall care plan
  • Support PCP‑led medication management by monitoring adherence, side effects, effectiveness, and patient concerns
  • Use a patient registry to track caseloads, follow up with patients, re‑engage individuals who fall out of care, and identify patients who are not improving
  • Collaborate with primary care providers and psychiatric consultants to adjust treatment plans when needed
  • Document patient progress, care coordination, treatment plans, and recommendations in the EHR
  • Complete relapse‑prevention planning for patients who reach remission or treatment goals
  • Educate patients and families about behavioral health conditions, treatment options, and available resources
  • Participate in regular psychiatric caseload consultations and interdisciplinary care discussions
What We’re Looking For

Required Qualifications
  • Master’s degree in a behavioral health field
  • Active New York license as an LMSW, LCSW, LMHC, LMFT, or equivalent eligible license
