Home Health Physical Therapist; PT

Travel Physical Therapist providing home-based care in Northern California. Earn up to $3,380 per week on a 13-week assignment, with flexible scheduling and full medical, dental, and vision benefits.

• Evaluate patients in their homes and develop individualized treatment plans

• Provide skilled physical therapy services to improve mobility, strength, balance, and overall function

• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques

• Monitor patient progress and modify treatment plans as needed

• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members

• Complete accurate and timely electronic documentation

• Maintain compliance with home health regulations and agency policies

• Doctor of Physical Therapy (DPT) or Bachelor's/Master's Degree in Physical Therapy from an accredited program

• Current California Physical Therapist License (or eligibility to obtain)

• Current BLS Certification

• Valid driver's license and reliable transportation

• Strong communication, organization, and time management skills

• Ability to work independently while managing a flexible schedule

• Home Health experience

• Earn up to $3,380 per week, based on experience

• 13-week travel assignment with potential to extend
