This position will be primarily onsite, allowing for direct and personal interaction with patients in a home care setting. You will collaborate with a multidisciplinary team composed of other physical therapists, occupational therapists, and speech-language pathologists, fostering an integrated approach to patient care. • Implement tailored treatment plans for patients, emphasizing mobility improvement, strength enhancement, and overall functionality, while meticulously documenting patient progress and adapting plans as necessary.
• Guide patients through individualized therapeutic exercises, stretches, and activities designed to promote physical well-being, alleviate pain, and enhance recovery.
• Educate patients on proper body mechanics, exercise techniques, and home-based exercise programs to support ongoing recovery and health maintenance.
• Administer therapeutic modalities—including electrical stimulation, ultrasound, and heat/cold therapy—while ensuring patient safety and comfort.
• Monitor and document progress, tracking improvements in range of motion, strength, and patient satisfaction to communicate effectively with team members.
• Prepare treatment areas and maintain equipment, ensuring that all resources are organized and ready for patient sessions.
• Engage with the interdisciplinary team to coordinate care plans, ensuring seamless communication and optimal patient outcomes.
• Provide emotional support to patients, encouraging a positive and motivating atmosphere that fosters their commitment to rehabilitation.
• Uphold the highest ethical standards in patient care by ensuring confidentiality and treating all patients with respect and dignity.

• A Bachelor's Degree in Physical Therapy from an accredited program.
• A valid Physical Therapist license in the state of New York.
