Travel Physical Therapist specializing in Home Health services, managing adult patients with chronic conditions, wound care, IV therapy, pleurx drainage, and wound vac care.

Responsibilities

Provide home-based therapy, handle approximately six patient visits daily, perform assessments using OASIS-E, chart with Homecare Homebase, and utilize Samsung Note 9 phones. Work within York County and neighboring areas, with travel up to 100 miles as needed.

Qualifications

Bachelor's Degree in a relevant field
valid state licensure
BLS certification
at least 1 year of home health experience
valid driver’s license with insurance
proficiency in geriatric and chronic disease management
No float required

Shift & Work Environment

Day shift, 8 hours per day, minimum 40 hours weekly, focused solely on home health care. Scrub color required:
Grey.
