Remote Bilingual Transitions of Care NP​/PA

Devoted Health is seeking a seasoned Transitions of Care Advanced Practice Provider (NP/PA) to lead post-discharge care for high-risk patients across the U.S. in a fully remote model.

You will perform initial TOC visits, reconcile meds, order tests, and coordinate with an interdisciplinary team to prevent readmissions.

Required:

5+ years of geriatric patient care, licensure as RN/APRN or PA, and experience with telehealth.
