Cadence is hiring a Bilingual/Spanish Speaking Clinical Navigator (LPN) to support Medicare patients with chronic diseases. This role focuses on delivering proactive, patient-centered care through outreach, education, and coaching, ensuring that patients feel supported throughout their care journey.
This is a fully remote position with standard hours from Monday to Friday. Candidates must have an active LPN license and at least 5 years of clinical experience. The anticipated pay is $26 - $30 per hour, complemented by comprehensive health benefits.
