Cadence is hiring a bilingual Spanish-speaking Clinical Navigator (LPN/LVN) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs.
As part of a clinical team, you will conduct proactive outreach, coach patients, coordinate care, and document interactions in the Cadence platform. Strong Spanish skills and multi-state licensure are required for this remote role.
