Cadence is a clinical AI company delivering virtual chronic care for Medicare patients enrolled in Cadence care programs. We are hiring a bilingual Spanish-speaking Clinical Navigator (Medical Assistant) to provide proactive outreach and care coordination within Cadence care programs.
You will educate patients, coach on self-management, perform assessments, document interactions in the Cadence platform, and collaborate with clinicians to resolve barriers and close gaps in care.
