• Conduct high-volume phone outreach and manage the Scheduling inbox with timely follow-up.

• Educate patients on proactive chronic care support and address questions or concerns.
• Accurately document patient interactions, outreach attempts, and enrollment outcomes.
• Collaborate with care teams and internal partners to ensure a seamless patient experience.

• Bilingual English/Spanish with prior care management experience.
• Strong call center and telephone communication skills.
• Comfort using technology, with reliable internet, personal laptop, and smartphone.
