The Core Program Manager – ECM provides operational and practice leadership in support of program direction established by the Program Director across Aspiranet’s CalAIM service continuum, including Enhanced Care Management (ECM), Community Supports (CS), and Community Health Workers (CHW) services. Serving as the workforce operations lead under the Program Director’s leadership, this role focuses on day-to-day service delivery oversight, workforce development, field-based support, and practice quality monitoring to ensure services are delivered consistently and effectively.
This role is accountable for day-to-day service delivery performance and operational consistency across assigned teams.
The Core Program Manager acts as a key bridge between program design and frontline execution, ensuring staff have the training, tools, clinical guidance, and operational structure needed to provide high-quality, person-centered care to members with complex health and social needs.
🔍 Key Responsibilities Practice Implementation & Service Model Fidelity- Ensure consistent implementation of ECM, CS, and CHW service models, workflows, and care coordination practices based on program models, policies, and established workflows.
- Promote fidelity to program models while supporting individualized, member-centered care.
- Conduct field-based support, case consultations, and service delivery reviews.
- Identify variations in practice and implement coaching, practice supports, and workflow improvements to strengthen service consistency and quality.
- Serve as a secondary level of review for complex care planning and elevate program design, policy interpretation, or payer-related issues to program leadership.
- Promote trauma-informed, culturally responsive, and equity-focused service delivery practices in daily practice.
- Lead onboarding and ongoing training to reinforce service standards and expectations set at the program leadership level.
- Provide structured coaching and reflective supervision to strengthen practice skills.
- Support the development of staff and emerging supervisors through skill development and practice guidance.
- Foster a culture of continuous learning, accountability, and professional growth.
- Provide direct and/or functional supervision, coaching, and practice support to assigned staff.
