Overview
The Manager, Care Management leads efforts to advance Quartz's care management programs, encompassing the "whole-person care model" while ensuring cross-team collaboration. This role will manage programs and processes including, but not limited to: capacity building, program promotion and evaluation, and internal business coordination.

- Serve as a leader and advocate for Quartz Care Management programs, with the ability to make an impact and influence the care and well-being of Quartz members directly.
- Starting salary range, based on skills and experience: $91, + robust total rewards package.
- Partner with Care Management leadership to oversee initiatives that strengthen and enhance the Care Management programs, services, workflows, and outcomes.
- Monitors and communicates regulatory and accreditation requirements, ensuring organizational readiness and compliance.
- Consults with and influences leadership and key stakeholders across the organization to support collaboration, education, program growth, and achievement of strategic goals.
- Monitors departmental performance, operational efficiency and outcomes, key performance indicators (KPIs), and return on investment (ROI) metrics, reporting results and recommendations to Executive Leadership.
- Serves as a knowledgeable and professional representative of the Care Management department and Quartz in internal and external meetings, committees, and stakeholder engagements, as requested.
- Promotes the visibility, contributions, and accomplishments of Care Management staff across the organization in support of collaboration and organizational engagement.
- Develops and delivers presentations, reports, and communication materials to internal and external audiences to highlight Care Management initiatives, successes, opportunities, and outcomes.
- Directs, develops, and leads Care Management Leads and staff, fostering a culture of accountability, engagement, collaboration, and continuous improvement.
- Models organizational values and promotes a high-performance, mission-driven culture aligned with enterprise priorities.
- Oversees talent management activities including recruitment, onboarding, staff development, performance management, succession planning, and retention.
- Ensures organizational strategies, goals, priorities, and expectations are effectively communicated and consistently reinforced with staff.
- Promotes professional growth and workforce development through training, coaching, mentoring, and skill-building opportunities.
- Proactively identifies and removes operational barriers, addresses staff concerns, and supports successful implementation of departmental and organizational initiatives.
- Ensures staff adherence to organizational policies, procedures, regulatory requirements, and performance standards.
- Participates in financial forecasting, budget planning and management, and resource allocation activities in collaboration with leadership and management teams.
- Bachelor's degree in social sciences, health education, nursing, or a related health field, with at least 5+ years of experience in healthcare-related field or equivalent mix of relevant experience and formal education.
- 1+ year(s) of formal people-leadership experience, and 3+ years of case management experience.
- Accountability for team performance and results – continually expands departmental capabilities using sound judgment and interpretation. Proven ability to train, supervise and inspire others.
- Demonstrated ability to develop and manage initiatives that deliver results while fostering teamwork and collaboration.
- Recognized subject matter expert in care management across clinical, social, behavioral, and population health domains with strong understanding of government and regulatory requirements.
- Excellent organizational skills with the ability to manage priorities and meet deadlines.
- Strong initiative, problem-solving, critical thinking, and decision-making skills.
- Excellent written and verbal communication skills; able to clearly convey ideas, instructions, and feedback to staff, members, and partners.
This position can work 100% remotely or from our offices in Madison, WI or Onalaska, WI in a hybrid or full-time on-site arrangement, if preferred. Hardware and equipment will be provided by the company, but candidates must have access to high-speed, non-satellite Internet to successfully work from home.
