Manager Care Management – Social Work
Location:
Everett, Washington (just outside Seattle).
Employment type:
Full‑time.
Salary: $113,000 to $178,630 per year (depending on experience).
Relocation assistance may be available.
Key Responsibilities- Manage care coordination for patients within a 500‑bed acute care hospital setting.
- Provide leadership and oversight for a team of social workers and care managers.
- Develop and implement case management protocols to improve patient outcomes.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Minimum 6 years of experience in social work, case management or hospital leadership.
- Experience in acute care case management and care management leadership.
- Strong organizational, communication and analytical skills.
- Proficiency in healthcare IT systems and documentation.
- Ability to lead, train and mentor a multidisciplinary team.
