The Wellness Center for Older Adults, a nonprofit, is accepting applications for a Street Outreach Social Worker/Case Worker to serve in communities across Collin County, Texas.

THE AGENCY

Established in 1978, The Wellness Center for Older Adults is Collin County’s premier agency in serving and addressing the needs and concerns of our older population. Core programming focuses on health services, counseling, and a variety of social and supportive programs for seniors and caregivers.

JOB DESCRIPTION SUMMARY/OBJECTIVE
  • The role involves providing supportive case management services to older adults experiencing unsheltered homelessness through visits to parks, emergency or day shelters, encampments, libraries, and other locations where unsheltered individuals congregate. It is not uncommon for unhoused individuals to have behavioral/mental health issues and/or substance use disorders. The preferred candidate should have experience working with individuals with these at-risk individuals.
  • WCOA works closely with Housing Forward and fellow service providers to provide case management and housing navigation, and community integration services, including, but not limited to, assessing homeless persons to identify and connect them to needed supportive services and assisting with obtaining Federal, State, and Local benefits.
  • Bachelor’s level professional with a degree in Social Work, Gerontology, or a related field is preferred. Experience with housing and Adult Protective Services (APS) is a plus. Bilingual is a plus.
  • This position is grant-funded and contingent on funding availability.

WORK ENVIRONMENT

This job operates in a professional office environment. This role routinely requires the use of standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

The job operates primarily in the field, with only about 20% of the time in the office.

  • Remote/telecommute work is NOT an option.

While performing the duties of this job, the employee is required to communicate actively. The employee is frequently required to stand, walk, and reach with their hands and arms. This position requires occasional lifting of office products and supplies up to 20 pounds.

POSITION TYPE/EXPECTED HOURS OF WORK

The job is a full-time position. Scheduled hours and days of work are flexible, subject to agreement with the executive director.

TRAVEL

Travel in and around Collin County, Texas, is required for street outreach services.

REQUIRED EDUCATION AND EXPERIENCE
  • Bachelor’s Degree in Social Work or related field with 2 years of experience.
  • Must be able to work a relatively flexible schedule to accommodate agency needs.
  • Must have reliable transportation and current insurance.
  • Must pass a pre-employment background check
  • Must be of high moral, ethical, and professional character.
  • Ability to lift at least 20 pounds.
  • Ability to engage and effectively communicate with persons from diverse cultures and communities.
  • Skills in organizing work responsibly within an environment with multiple challenges, priorities, and time frames.
