The Consortium Manager will convene, facilitate, and sustain a robust Community Consortium composed of diverse, multi-sector partners that provide guidance and support for Healthy Start (HS) activities under the direction of the Project Director. This position is responsible for leading the implementation of action plans designed to improve perinatal outcomes within the project area and for fostering strategic partnerships that address participant needs.

The primary goal of this role is to facilitate and support a coalition of partners that drives systems change, strengthens community collaboration, and implements initiatives that improve birth outcomes and advance maternal and infant health.

The Consortium Manager will collaborate with clients, healthcare providers, mental health service organizations, WIC, SNAP, TANF, and agencies focused on housing and food insecurity, as well as key stakeholders including Title V programs, public health departments, hospitals, health centers, substance use treatment agencies, community leaders, and other service organizations.

PRIMARY RESPONSIBILITIES Consortium Leadership and Partnership Management
  • Lead the development, coordination, and ongoing operation of the Healthy Start Community Consortium in alignment with program goals and funder requirements.
  • Identify, engage, and maintain strategic relationships with healthcare providers, public agencies, community organizations, and other partners working to improve maternal and infant health outcomes.
  • Ensure meaningful participation of Healthy Start participants and community members with lived experience, consistent with program requirements.
  • Serve as the primary liaison between the Consortium, community partners, and Healthy Start program leadership.
  • Represent Global Communities and Healthy Start at community events, coalitions, health fairs, and partner meetings.
Strategic Planning and Systems Change
  • Plan and facilitate monthly Consortium, workgroup, and subcommittee meetings, including agendas, materials, decisions, and follow‑up actions.
  • Lead the development, implementation, and monitoring of Consortium strategic action plans addressing community or environmental factors that affect birth outcomes.
  • Guide work groups and partners in establishing priorities, performance measures, responsibilities, and timelines.
  • Support the implementation of systems‑level actions that strengthen service coordination and improve perinatal outcomes.
  • Provide recommendations to the Project Director based on Consortium feedback, community needs, emerging trends, and implementation progress.
  • Participate in Communities of Practice and other collaborative learning opportunities.
Community Collaboration and Engagement
  • Strengthen cross‑sector partnerships and referral pathways to improve participant access to healthcare, behavioral health, nutrition, housing, education, and other community services.
  • Identify service gaps, barriers, and opportunities for improved coordination among community partners.
  • Lead the planning and implementation of required Consortium activities in collaboration with program staff and community partners.
  • Promote participant voice and lived experience in Consortium planning, decision‑making, and program improvement.
Communication and Outreach
  • Develop and maintain Consortium communications, including newsletters, surveys, referral agreements, presentations, and engagement materials.
