Gemeente Zeist zoekt een Projectmanager toegang CJG en Sociaal Team voor 24 uur per week, tijdelijk voor de duur van één jaar, met focus op ontwikkelen en borgen van een toekomstbestendig Contactpunt CJG.
Je werkt samen met huisartsen, scholen en maatschappelijke organisaties, stuurt op triage en betere toegang sociaal domein, en zet in op cultuurverandering naar normaliseren.
Je hebt HBO+/WO-niveau en ervaring als projectleider binnen het sociaal domein.
