Per Diem up to 8 hours per week. Workdays, Variable work schedule including shifts on weekends and evenings)

The Lead Promotora(or) will conduct outreach, education, system navigation, and community engagement activities while supporting the implementation of the assigned project work plan and team goals. This position will build relationships with community-based organizations and coordinate mentorship activities that connect experienced and emerging programs focused on diabetes prevention and management. The Lead Promotora(or) will facilitate peer learning, provide ongoing support to partner organizations, and collaborate with program staff and community partners to strengthen program efforts and achieve project goals.

• Understand and follow the work plan provided by the Supervisor for the assigned project.
• Communicate the supervisor’s daily plan of activities to the team.
• Maintain and organize all reports created by the team and provide them to the supervisor.
• Report emergencies or other issues that occur in the field to the supervisor to manage.

• Understand and work with the team to meet the requirements and restrictions for specific line items outlined in the project budget including but not limited to printing, outreach materials, office supplies, and equipment; communicate any potential budget issues to the Supervisor.
• Provide and transport equipment and supplies needed by the team for day-to-day and events and activities.
• Provide technical or logistical support to the team in using virtual platforms and conducting in-person outreach as needed.

• Provide the team with information and referrals to additional resources in support of the needs of the community.
• Regularly review and update program resources to ensure they are current, accurate, consistent, and user-friendly facilitating successful referrals for program participants.

• Identify and secure potential sites, groups, and community-based organizations to provide information about the assigned program and its services for community members in the assigned region.
• Conduct outreach and educational activities to inform community members in culturally and linguistically relevant ways.
• Conduct outreach and implement mentorship activities with partner organizations offering diabetes prevention and management programming.
• Engage and recruit community-based organizations and facilitate connections between experienced and emerging programs to support peer learning, knowledge sharing, and collaboration.
• Coordinate and/or implement mentorship activities, including facilitating connections, supporting peer learning opportunities, and providing ongoing guidance and technical assistance to participating organizations.
