Home Health Registered Nurse

Overview

We are seeking a skilled and compassionate Registered Nurse (Home Health) to join our client’s expanding team in Morton Grove, Illinois. In this rewarding role, you will provide high-quality, personalized nursing care to patients in their homes—helping them maintain their independence, comfort, and quality of life. The ideal candidate will possess strong clinical skills, exceptional attention to detail, and a genuine commitment to patient-centered care.

• Conduct comprehensive patient assessments and provide skilled nursing care in home settings
• Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
• Educate patients and families on medication management, disease processes, and post-care instructions
• Monitor and evaluate patient progress, adjusting care plans as needed
• Maintain accurate, timely clinical documentation in compliance with agency standards and state regulations
• Coordinate with healthcare providers to ensure seamless continuity of care

• Monday through Friday, daytime hours
• On-call rotation: 1 weekend per month, 2–3 on-call days per month, and 2 holidays per year
• This is a Pay Per Visit position

• Local travel within the Morton Grove area (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Illinois
• Minimum of 1 year of professional RN experience (home health preferred)
• Current BLS or ACLS certification through the American Heart Association
• Valid driver’s license, reliable vehicle, and current auto insurance

• Prior experience in home health or community-based care
• Strong communication, organization, and critical thinking skills
