We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our client’s expanding team in Riverside, Illinois. In this role, you will deliver professional nursing care to patients in the comfort of their own homes, promoting recovery, independence, and overall well-being. The ideal candidate is a motivated, organized clinician with strong communication skills and a passion for home-based patient care.

• Conduct comprehensive assessments and provide skilled nursing care in patients’ homes
• Develop and update individualized care plans in coordination with physicians and interdisciplinary team members
• Educate patients and families on treatment plans, medications, and disease management
• Monitor and document patient progress, adjusting care plans as necessary
• Ensure compliance with all clinical and regulatory standards through accurate and timely documentation
• Collaborate effectively with the healthcare team to ensure high-quality, continuous patient care

• Monday through Friday, day shift
• 1 weekend per month and 2–3 on-call days per month
• 2 holidays per year
• This is a Pay Per Visit position

• Local travel required within the Riverside area (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Illinois
• Minimum of 1 year of professional RN experience (Home Health experience preferred)
• Current BLS or ACLS certification through the American Heart Association
• Valid driver’s license, reliable vehicle, and current auto insurance

• Prior experience in home health or community-based nursing
• Excellent clinical, communication, and time management skills
