Advanced Maintenance Solutions, LLC is looking for a Visiting Registered Nurse (RN) Full-time/Part-time with flexibility for work schedule.
Pay Range$60.00/hr - $100.00/hr (actual pay based on skills and experience). PRN rates per visit with mileage reimbursement, iPads with company paid cellular data, full-time positions with benefits.

BriefJob Description
The registered nurse plans, organizes, and directs home care services emphasizing health education and experience. The RN Case Manager travels to a patient’s home to administer services dictated by their physician and At Home Quality Care, helping patients maintain independence and quality of life.
Essential Job Functions / Responsibilities- Assess and chart observations of the patient's condition and needs at each home visit.
- Complete evaluation tasks, including medication reviews and vital signs.
- Initiate the Plan of Care and update as patient status changes.
- Provide education to patients and family on home health procedures.
- Develop a care plan establishing goals based on nursing diagnosis.
- Administer medications and treatments as prescribed by the physician.
- Provide health care instructions per assessment and plan of care.
- Identify discharge planning needs and prepare patient and family for discharge.
- Assume case management responsibilities when assigned by Clinical Supervisor.
- Coordinate with physicians, physical therapists, occupational therapists, MSWs, and others.
- Perform supervisory visit with CNA once every two weeks.
- Prepare clinical notes and update primary physician as needed.
- Communicate patient needs and changes to physician and office.
- Coordinate with community health personnel to align care plan.
- Report updates, concerns, and issues to home office.
- Graduate of an accredited school of nursing.
- Current licensure in Illinois and current CPR certification.
- Management experience not required, but responsible for supervising home health aides.
- Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills, nursing skills per competency checklist.
- Ability to perform prolonged walking or standing, lifting, transferring patients, and lifting supplies and equipment.
- Must be a licensed driver with an insured automobile in good working condition.
- Experience with Axxess highly desired.
