Obesity Medicine Nurse Practitioner

Form Health is a virtual obesity medicine clinic delivering multidisciplinary evidence-based obesity treatment through telemedicine. Our care team includes board-certified physicians, advanced practice professionals, and Registered Dietitians. Through our proprietary mobile app, patients engage in regular video visits, text messaging, photo journaling, digital data transmission, and customized educational materials. • Provide longitudinal telehealth care for patients with obesity as a Nurse Practitioner, collaborating with physicians and registered dietitians to deliver team-based care.
• Apply evidence-based obesity management, including lifestyle counseling and pharmacotherapy.
• Deliver care through virtual visits and coordinate care using the Form Health app (video visits, messaging, photo journaling, data transmission, educational materials).
• Document clinical encounters and contribute to care planning with the multidisciplinary team while upholding high-quality medical care and patient empathy.

This is a full-time, remote position. You would collaborate with physicians and registered dietitians to deliver exceptional team-based care for patients with obesity.

• Nurse Practitioner degree, with experience practicing Obesity Medicine, including effective evidence-based lifestyle counseling and pharmacotherapy.
• Licensed to practice and prescribe in at least one of the following states:
  MA
  , NY
  , TX
  , NJ
  , and/or IL
  .
• 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2+ years of obesity medicine experience in a primary care/internal medicine setting.
• Excellent interpersonal skills for patient and team interactions.
• Committed to the highest quality of medical care and an evidence-based approach to obesity management.
• Experience conducting telemedicine visits with a strong, professional web-side manner.
• Bilingual candidates with medical fluency in Spanish are highly preferred.
