Registered Nurse (RN)/Case Manager

Reports To: Director of Nursing

Pay: $60.00 – $100.00 per visit (PRN rates). Mileage reimbursement, iPad with company cellular data, and opportunities for full‑time positions with benefits.

The RN Case Manager plans, organizes, and directs home care services emphasizing health education and experience. The RN Case Manager travels to patients’ homes to administer services dictated by the physician and At Home Quality Care, helping patients maintain independence and quality of life.

• Assess and chart observations of the patient’s condition and needs at each home visit.
• Complete evaluation tasks, including medication reviews and vital signs.
• Initiate and update the Plan of Care and revise it as patient status and needs change.
• Provide education to patients and family members on proper home health procedures and strategies.
• Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
• Administer medications and treatments as prescribed by the physician.
• Provide health care instructions to the patient as appropriate per assessment and plan of care.
• Identify discharge planning needs as part of the care plan and prepare patient and family for discharge.
• Act as Case Manager when assigned by the Clinical Supervisor and coordinate patient care for assigned caseload.
• Coordinate with physicians, physical therapists, occupational therapists, MSWs, and other individuals in the patient care plan. Perform a supervisory visit with a CNA (when part of the care plan) once every two (2) weeks.

• Prepare clinical notes and update the primary physician when necessary.
• Communicate with the physician and office regarding the patient’s needs and report any changes; obtain/receive physician orders as required.
• Communicate with community health‑related persons to coordinate the care plan.
• Report with the home office on any updates, concerns, and issues regarding the patient.

Position Qualifications
• Graduate of an accredited school of nursing.
• Current licensure in the state of Illinois and current CPR certification.
• Responsible for supervising home health aides (management experience not required).
• Excellent observation, verbal and written communication skills, problem‑solving skills, basic math skills, and nursing skills per competency checklist.
• Must be able to lift, position, or transfer patients; lift supplies and equipment; considerable reaching, stooping, bending, kneeling, or crouching; visual acuity and hearing to perform required nursing skills.
• Must be a licensed driver with an automobile insured under the state of IL. Automobile is in good working order.
• Axxess experience is highly desired.
