HELP OUR HOMEBOUND PATIENTS STAY OUT OF HOSPITALS!!!!

Seeking Registered Nurses to administer skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.

• Current Illinois License and graduate of an approved school of nursing
• Must have prior home health care experience
• Ability to exercise initiative and independent judgement
• We pay per visit upon completion of clinical charting.
• BILINGUAL RN's a plus*
• Servicing non-COVID patients, COVID positive patients, and PUI cases
• Chicago and surrounding suburbs
• Active license required
• Visit patients in their homes and follow care plan
• Ability to use EMR on phone a must - AXXESS experience is a plus!!!!
• Clinicians must use own transportation
• Charting knowledge per CMS requirements
