The RN Care Manager is a fully remote contract focused on supporting patients with diabetes through proactive care coordination, education, and clinical triage. This nurse serves as a primary clinical contact for patients, monitoring health data, identifying changes in condition, and escalating concerns to treating providers in a structured telehealth environment.
This role is ideal for an RN with hands-on experience in diabetes management who thrives in virtual care and patient education.
Job Responsibilities- Manage a caseload of patients with diabetes, serving as the primary escalation point for changes in condition or increased care needs.
- Conduct proactive patient outreach based on incoming monitoring alerts, glucose trends, and other clinical indicators.
- Assess physiological data including blood glucose readings, symptoms, and related comorbidities, taking timely action when values fall outside established parameters.
- Provide patient education focused on diabetes self-management, medication adherence, lifestyle modification, and recognition of warning signs.
- Identify signs of worsening condition and escalate concerns to MD, NP, or PA using standardized clinical communication protocols.
- Support non-licensed staff in executing care management tasks related to diabetes monitoring and patient follow-up.
- Follow established care protocols and document assessments, interventions, and any necessary deviations accurately and thoroughly.
- Coordinate additional resources as needed, including device support, care adjustments, or referrals.
- Assist with coverage across other clinical programs when needed to support urgent staffing or patient care needs.
- Perform other related duties as assigned by Team Leads or Project Managers.
- Active Registered Nurse license in good standing.
- Minimum of 2 years of clinical nursing experience.
- Direct experience managing patients with diabetes, including assessment, education, and ongoing condition monitoring.
- Graduation from an accredited registered nursing program.
- At least 6 months of experience in telephonic triage and patient education.
- Ability to obtain and maintain multi-state licensure and required continuing education.
- Active licensure in New York
- Strong clinical judgment and critical thinking skills, particularly in diabetes management.
- Excellent verbal and written communication skills with the ability to educate patients at varying health literacy levels.
- High proficiency in telephonic assessment and independent triage, including escalation to treating providers.
- Ability to develop patient-centered care plans tailored to individual needs and social determinants of health.
- Comfort working across multiple technology platforms, including telehealth and remote monitoring systems.
- Ability to follow and initiate standing orders as appropriate.
- Strong collaboration skills and clear communication with interdisciplinary care teams.
- Bilingual in English and Spanish.
- Experience managing diabetes in a telehealth or remote monitoring environment.
- Ability to obtain additional state endorsements as needed.
